I give permission for my/my child’s photograph and/or name to be published in all forms of media including Peninsula Softball Club Website, Facebook and Instagram (but not limited to) official programs, newsletters and other websites.

I understand that my I/my child may be included in photographs and/or video footage throughout the season and this material may be used by Peninsula Softball Club and/or Manly Warringah Softball Association for advertising and all forms of media.
